WCPH currently accepts the following insurances for patients of all ages: Aetna (not HMO), Anthem Blue Cross & Blue Shield, Cigna, CoreSource, EBMS, FrontPath, Golden Rule, Healthscope, MedBen, Medical Mutual/Mutual Health, Meritain, Multi-Plan, NWOHP, Ohio Health Choice, UMR, and United Health Care for immunizations and preventative health services.
WCPH also accepts Medicaid and Medicare for certain vaccines and services. Please call 419-294-3852 for appointments and/or to talk to the billing specialist for questions regarding coverage.
Wyandot County Public Health is a Vaccines For Children provider. To qualify for this program, individuals 18 and under must have Medicaid or no insurance, or be of the Native American or Alaskan Native community.
Insurances accepted under the VFC program include: Buckeye, CareSource, Molina (no marketplace), Paramount (no marketplace), United Health Care Community Plan.
Our agency's goal is to have 90% of all children "up to date" with their immunizations by the age of 24 months. Education efforts included participation at health fairs, speaking at clubs and organizations, newspaper articles, radio public service announcements, and distributing brochures and other educational materials. Immunizations are offered at WIC appointments to reduce vaccine preventable diseases.
The Ohio Department of Health maintains the Ohio Impact Statewide Immunization Information System (ImpactSIIS), an online tool that allows providers to record immunizations/vaccinations administered in the state of Ohio. If you are seeking to obtain a copy of your immunization records from the state system, it is important to know that such records are protected by medical confidentiality laws and a written request must be submitted to receive them.
Please complete an Immunization Record Request Form (click here to download) to request a copy of your immunization record and submit to Wyandot County Public Health using the steps noted on the form.
*It is possible that if Wyandot County Public Health did not administer all of the vaccines you have received, your record may not be complete. Medical providers in Ohio are not required to report immunizations to the State Immunization Registry. Please contact your healthcare provider who provided your vaccination.
